Postmarks

Postmarks are acceptable as proof of timely filing when the report is postmarked by the U.S. Postal Service no later than the day prior to the filing deadline.

Campaign Finance Forms

• Campaign Finance Report - For activity exceeding $250 during a reporting period.
• Campaign Finance Statement - For activity not exceeding $250 during a reporting period.
• Unsworn Declaration for Campaign Finance Reports - To accompany signed campaign finance reports (<$250) in lieu of notarization.
• Unsworn Declaration for Campaign Finance Statements - To accompany signed campaign finance statements (>$250) in lieu of notarization.
• Late Contributions - 24 Hour Report - For reporting contributions or pledges of $500 or more during the 24-hour reporting window.
• Committee Registration Statement - To be completed should you wish to establish a political committee.
• Authorization to Receive Funds - This form should accompany Committee Registration Statements when the committee is a candidate's authorized committee.
• Campaign Finance Reporting Law - A campaign finance manual for candidates & political committees.

Candidates and their authorized political committees file reports in the office where their nomination documents are filed. If the candidate's reports are filed with the Secretary of the Commonwealth, a copy of the reports filed by the candidate and the authorized committee must also be filed with the County Board of Elections in the county in which the candidate resides.
