The documents listed below are also required when filing for support in an UIFSA (interstate) case.

• Copies of birth certificates for children, even if they are not part of this support action.
• Copies of Acknowledgements of Paternity if from any state other than PA.
• If the Plaintiff pays support for any additional children, provide copies of the order and payment record and/or proof of payment if available.  
• If the Defendant pays support for any additional children, provide copies of the order and payment record and/or proof of payment if available.
• Copies of any parenting time and custody order/agreement. 
• Copies of any divorce order or legal separation if the children are mentioned.
• Paystubs.
• Child care expense verification (broken down per child).
• Copies of medical insurance cards.
• Verification of any extraordinary medical expenses for the children.
• Verification of any unreimbursed medical expenses for the children.
• Verification of any ongoing medical expenses for the children.
