Case Management Services

The Case Management unit can help you access treatment and services related to the goal of addiction recovery. Case Management is a voluntary service, offering you the benefit of having an advocate who can coordinate services that ensure progress toward your recovery goals. A Case Manager can help in the following ways:

Screening

Screening is the first step in identifying the presence of a possible substance abuse problem. Basic information is collected in order to determine any immediate care needs which may result in a referral to a detoxification or other medical facility. If no immediate care needs are identified, an evaluation is then scheduled.

Evaluation

An evaluation involves meeting with a  case manager privately to identify the need for drug and alcohol treatment, to decide what level of care is appropriate, and to identify what other needs or concerns may affect recovery. The case manager will also determine eligibility for Commission funding for treatment.

Referral

If treatment is appropriate and funding eligibility has been determined, the case manager will make a referral to an appropriate treatment facility and monitor your progress. If additional services are needed upon completion of treatment, the case manager will help connect you with the resources you need.

Support for Families

Addiction is a family disease. With that in mind, the Commission is committed to providing support to families of those who struggle with addiction.  Educating yourself about the disease of addiction is key when helping a loved one; taking care of yourself is equally important.
